We have two Social Prescribers who work across the practises - Tessa and Nuala.
Social Prescribing is recognising that people’s health and wellbeing are determined mostly by a range of social, economic, and environmental factors. It seeks to address people’s needs in a holistic way. It also aims to support individuals to take greater control of their own health.
How do they help you:
- Support to make positive changes in your life
- Help with housing, benefits, and financial problems
- Referring/signposting you to community organisations such as support groups and creative activities
- Referral to professional support
- Supporting you to communicate and/or identify barriers
- Listening and giving you the time you need to talk
- Advice on jobs, training, and education
- Researching the relevant support available to you
- Advocating to help make sure your views are listened to and voice is heard
